Event Details Questionnaire

To request The Brooklyn Tabernacle Singers to minister at an event, please download our Event Details Questionnaire here, and fax it to the Music Department at (718)290-2051.

We must receive your Event Details Questionnaire, along with an official letter from a Senior Pastor and/or Ministry Leader. Please allow three (3) to five (5) weeks for a response.

Prayer Request Form

Please give us a brief description of your prayer request. Once received, our Prayer Band will petition the Lord on your behalf for the next 30 days. (Please bear in mind that not all prayer requests will be displayed on the website prayer scroll below.)

Please be sure to send us your praise report when the Lord answers your request.
